Complete this form as required by the Kane County Code (Chapter 30-1) 

State law requires registration as described below:​

"Every natural person and every entity required to register under this Act shall annually submit the registration required by this Section on or before each January 31."

​Mail the form and required supplemental documentation to the Kane County Clerk's Office, 719 S. Batavia Ave., Bldg. B, Geneva, IL 60134

The form can be brought in to the the Geneva office as well.
